Medicare Basics

Medicare A, B, C & D

Medicare has 4 parts: Part A - Inpatient, Part B - Outpatient, Part C - Medicare Advantage and Part D - Prescription Drug Coverage

Part A is all inpatient services including: hospital, skilled nursing, home health and hospice services.  There is not premium if someone has worked the equivalent of 10 years and paid into Social Security.

Part B includes all medical services on an outpatient basis like doctor office visits, therapies, treatments and testing, home health care, durable medical equipment and many preventive services.  There is a monthly premium for Part B and most people pay $174.70/month. People pay more if they are high wage earners and some people pay less if they qualify for Medicaid.

Part A & Part B are considered "Original Medicare" and are managed through the Social Security Administration.

Part D covers prescription drugs. Rather than getting coverage through Social Security, Part D Rx plans are purchased from a private company that has contracted with Medicare to offer Part D prescription drugs.  This can be a "stand alone" prescription drug plan to accompany a Medicare Supplement plan or can be part of a Medicare Advantage plan. If someone fails to get Part D on time, there may be a Late Enrollment Penalty (LEP).

Part C is Medicare Advantage plans. These are private companies that contract with Medicare to offer a health plan that includes all Medicare Part A and Part B services, often cover Part D and usually offer more services like vision and dental.  Medicare Advantage plans vary by where you live and may include a monthly premium.

Original Medicare Costs

Medicare Part A has no cost if you worked the equivalent of 10 years and paid into Social Security. If you don't get premium free Part A you can pay up to $505 monthly.

Medicare Part B 2024 monthly premium for most people is $174.70/month. 

  • When you receive Social Security it is taken from your payment automatically. 
  • If you are not getting Social Security you are billed quarterly or can set up an automatic monthly payment through the www.medicare.gov website. 
  • If you are a high wage earner you may pay IRMAA (Income Related Monthly Adjustment Amount). On the other hand, if you are under the Federal Poverty Level (FPL) you may qualify for assistance though Medicaid or Low Income Assistance (LIS) and have your premium covered or reduced.


Medicare Deductibles and other Costs 2024

Part A - Hospital stay

  • $1,632 deductible per benefit period
  • $0 for the first 60 days of each benefit period
  • $408 per day for days 61–90 of each benefit period
  • $816 per “lifetime reserve day” after day 90 of each benefit period (up to a maximum of 60 days over your lifetime)

Part A -Skilled Nursing Facility stay

  • $0 for the first 20 days of each benefit period
  • $204 per day for days 21–100 of each benefit period
  • All costs for each day after day 100 of the benefit period

Part B Deductible

  • $240 before Original Medicare starts to pay

Part B Coinsurance 

  • Medicare covers 80% and you pay 20%
